What does “waning immunity” after COVID-19 vaccination mean?
Multiple studies observed that protection against SARS-CoV-2 infection declined during the months after mRNA vaccination, particularly as viral variants changed. Protection against hospitalization and severe disease generally remained more durable than protection against any infection. These estimates are time-, population-, and variant-specific and should not be treated as permanent vaccine-effect numbers.
Why estimates change
Vaccine effectiveness compares outcomes among groups under real-world conditions. It can change as immunity matures or declines, variants circulate, exposure patterns shift, and previously infected people accumulate in comparison groups.
Infection is not the same endpoint as severe disease
A study may find reduced protection against a positive test while still finding meaningful protection against hospitalization. When reading a percentage, ask which outcome, population, variant, interval after vaccination, and calendar period it describes.
Important limitations
Observational comparisons can be influenced by testing behavior, prior infection, health status, exposure, and differences between people vaccinated at different times. Statistical adjustment helps but cannot remove every difference.
