What can health records tell us about fibromyalgia?
In a matched electronic-health-record study, people diagnosed with fibromyalgia more often had several endocrine and metabolic diagnoses, including thyroid disease, diabetes, osteoporosis, and vitamin D deficiency. The findings show clinical overlap and may support thoughtful assessment, but they do not establish that an endocrine disorder causes fibromyalgia or that treating one will resolve it.
Study design
Researchers compared 9,232 people with fibromyalgia with 46,160 matched controls. They examined coded endocrine diagnoses recorded in routine care.
Main pattern
Hashimoto thyroiditis, Graves disease, type 1 and type 2 diabetes, Cushing syndrome, prolactinoma, osteoporosis, and vitamin D deficiency were among the conditions recorded more often in the fibromyalgia group.
What the association may reflect
- Shared biological pathways may contribute to some symptoms.
- Endocrine conditions can produce fatigue, pain, sleep disturbance, or cognitive complaints that overlap with fibromyalgia.
- People with chronic symptoms often receive more testing, increasing the chance that another condition is found.
- Diagnostic coding cannot fully capture severity, sequence, or the clinician’s reasoning.
What it does not mean
The study does not identify one endocrine explanation for fibromyalgia. It also does not support broad testing or supplements without an individualized clinical reason. The useful conclusion is that overlapping, treatable conditions should not be overlooked while fibromyalgia is assessed on its own clinical criteria.
